Endoscopic Sleeve Gastroplasty

Is Endoscopic Weight Loss Right For You?

Obesity is a complex metabolic disease of excessive fat accumulation associated with an increased risk to health. It contributes to a host of maladies including heart disease, hypertension, dyslipidemia, type II diabetes, osteoarthritis, non-alcoholic steatohepatitis (NASH), and sleep apnea.

Endoscopic weight loss therapies are designed for patients looking to achieve sustainable weight loss results and improve or cure their obesity-related medical problems. While diet and exercise alone can help, studies show it is hard to achieve sustained weight loss with these methods alone. Endoscopic weight loss therapies are an ideal complement to diet and lifestyle changes to achieve durable weight loss results as they provide some of the benefits of weight loss surgery while offering a lower risk profile.

Our Endoscopic Weight Loss Program specializes in treating adults who are overweight or with obesity, and who do not qualify for weight loss surgery or do not want surgery.

Endoscopic sleeve gastroplasty (ESG) is an innovative, popular, highly effective, and minimally invasive weight loss procedure. ESG is meant to complement diet and lifestyle changes in a patient’s weight loss journey. Patients undergoing the ESG procedure and following the medically supervised program designed by Reveal Weight Loss have demonstrated excellent weight loss results. The average weight loss ranges from 30 to 70 pounds. The potential weight loss can be better if the patient closely follows all the recommendations of the program.

What Is Endoscopic Sleeve Gastroplasty?

Endoscopic sleeve gastroplasty (ESG) is an endoscopic procedure that involves going into the stomach through the mouth using an endoscope with a suturing device attached and performing a gastric volume reduction. This technique is designed to make the stomach smaller and the stomach becomes banana shaped. ESG procedure is accomplished by using a series of sutures to fold the stomach on itself like an accordion, effectively reducing the stomach volume by 70 to 80 percent.

The result is the creation of a tube-shaped stomach similar to surgical sleeve gastrectomy but without surgery, incisions, scars, and removing stomach tissue. During the procedure, the patient is completely asleep with general anesthesia.

Who Is A Good Candidate?

A good candidate for ESG procedure is someone who:

  • Has had difficulty losing weight and/or maintaining weight loss with diet and exercise alone
  • Has a Body Mass Index (BMI) of 30 to 45
  • Does not want to undergo surgery for weight loss
  • Is willing and ready to follow a medically supervised program to achieve the best results

ESG procedure is not suitable for someone who has the following health conditions:

  • Large hiatal hernia
  • Previous gastric surgery
  • Heartburn symptoms despite taking heartburn medications
  • Prior open or laparoscopic bariatric surgery
  • Inflammatory diseases of the GI tract
  • Active ulcers in the stomach or small intestine
  • Variceal disease
  • Unwilling to participate in a medically-supervised diet and behavior modification program
  • Alcohol or drug addictions
  • Daily prescribed treatment with aspirin, anti-inflammatory agents, anticoagulants, or other gastric irritants
  • Currently are or may be pregnant or breastfeeding

How Does ESG Work?

During the ESG procedure, which involves a restrictive weight loss technique, the stomach is made smaller and shorter. Since the stomach is smaller, patients feel full quicker and consume fewer calories. Studies have also shown the procedure helps weight loss by delaying gastric emptying and altering hormones that control appetite and metabolism.

What Is Weight Loss Potential?

Endoscopic sleeve gastroplasty is one of the most effective endoscopic procedures for primary weight loss. It is important to understand that ESG is a tool to aid weight loss and must be used in conjunction with diet, exercise, and a behavior modification program.

One year after the ESG procedure, if the proper diet and exercise plan is followed, patients can expect to lose around 60% of their excess weight and 20 % of their total body weight. Patients who did the ESG procedure in NJ with the help of Reveal Weight Loss lost on average 30 to 70 pounds. The most recent data has shown the weight loss achieved with the ESG procedure can last for greater than 5 years.

Like other weight-loss procedures, endoscopic sleeve gastroplasty requires a commitment to a healthier lifestyle. Patients must make permanent healthy changes to their diet and get regular exercise to help ensure the long-term success of endoscopic sleeve gastroplasty.

What Are the Health Benefits?

ESG has been shown to improve obesity-related medical problems such as high cholesterol, diabetes, high blood pressure, obstructive sleep apnea, acid reflux, and joint pains, amongst other health benefits.

Some of the benefits and advantages of ESG procedure for your health include:

  • Does not leave scars and does not require incisions
  • Short recovery time
  • Minimally invasive procedure
  • Does not worsen acid reflux

What Are the Risks & Side Effects?

This is a very safe and reliable weight-loss solution and studies have shown a four-fold decrease in complications when compared to laparoscopic sleeve gastrectomy. ESG requires no surgery and the most common side effects last only for a few days. Typical symptoms after the procedure include abdominal cramping, nausea, bloating, and constipation. Your doctor will provide prescription medication to help alleviate these symptoms. The side effects of the ESG procedure usually last for about 3 to 5 days. Given the low caloric intake, some patients may experience fatigue for 1-2 weeks after the procedure, but you can return to work in 3 to 5 days. Major complications are rare.

What Can I Expect From The Procedure?

Preparation for ESG

Before conducting the procedure, the doctor will order bloodwork and EKG and prescribe anti-nausea medication to take before the procedure. Also, the night before the procedure, the patient will be asked to avoid eating or drinking.

During the ESG Procedure

  1. First, the patient is given anesthesia through an IV catheter.
  2. After the patient is asleep, an initial endoscopic examination is performed.
  3. A thin, flexible tube equipped with a camera and light is passed through the mouth into the esophagus, stomach, and small intestine.
  4. Then a special endoscope with a suturing device attached is passed into the stomach.
  5. A series of full-thickness sutures are placed through the stomach wall in the stomach body with the result being the creation of a sleeve-shaped stomach – similar to a surgical sleeve gastrectomy.

The sutures are made of a very durable and permanent material. The exact number of sutures needed will vary based on the size and shape of the stomach.

The suturing technique is intended not only to reduce the stomach diameter by approximately 70% to 80% but also to shorten it substantially through an accordion effect.

The procedure takes about 30 to 60 minutes and patients return home the same day, after recovering for 1-2 hours post-procedure.

Recovery after ESG

After the procedure, you may start clear liquids and remain on some form of a liquid diet for two weeks. Then there is progression to small semi-solid meals over the next 2 weeks and eventually to a regular healthy diet. Normally most patients go back to their everyday activities within three to five days after the ESG procedure. Patients will also continue to follow up with our multidisciplinary team of experts. One full year of follow-up visits with the physicians and with the nutritionist/dietician is included in our Reveal Weight Loss program.
