Endoscopic sleeve gastroplasty (ESG) is an endoscopic procedure that involves going into the stomach through the mouth using an endoscope with a suturing device attached and performing a gastric volume reduction. This technique is designed to make the stomach smaller and the stomach becomes banana shaped. ESG procedure is accomplished by using a series of sutures to fold the stomach on itself like an accordion, effectively reducing the stomach volume by 70 to 80 percent.

The result is the creation of a tube-shaped stomach similar to surgical sleeve gastrectomy but without surgery, incisions, scars, and removing stomach tissue. During the procedure, the patient is completely asleep with general anesthesia.

Who Is A Good Candidate?

A good candidate for ESG procedure is someone who:

  • Has had difficulty losing weight and/or maintaining weight loss with diet and exercise alone
  • Has a Body Mass Index (BMI) of 30 to 45
  • Does not want to undergo surgery for weight loss
  • Is willing and ready to follow a medically supervised program to achieve the best results

ESG procedure is not suitable for someone who has the following health conditions:

  • Large hiatal hernia
  • Previous gastric surgery
  • Heartburn symptoms despite taking heartburn medications
  • Prior open or laparoscopic bariatric surgery
  • Inflammatory diseases of the GI tract
  • Active ulcers in the stomach or small intestine
  • Variceal disease
  • Unwilling to participate in a medically-supervised diet and behavior modification program

What Can I Expect From The Procedure?

Preparation for ESG

Before conducting the procedure, the doctor will order bloodwork and EKG and prescribe anti-nausea medication to take before the procedure. Also, the night before the procedure, the patient will be asked to avoid eating or drinking.

During the ESG Procedure

  1. First, the patient is given anesthesia through an IV catheter.
  2. After the patient is asleep, an initial endoscopic examination is performed.
  3. A thin, flexible tube equipped with a camera and light is passed through the mouth into the esophagus, stomach, and small intestine.
  4. Then a special endoscope with a suturing device attached is passed into the stomach.
  5. A series of full-thickness sutures are placed through the stomach wall in the stomach body with the result being the creation of a sleeve-shaped stomach – similar to a surgical sleeve gastrectomy.

The sutures are made of a very durable and permanent material. The exact number of sutures needed will vary based on the size and shape of the stomach.

The suturing technique is intended not only to reduce the stomach diameter by approximately 70% to 80% but also to shorten it substantially through an accordion effect.

The procedure takes about 30 to 60 minutes and patients return home the same day, after recovering for 1-2 hours post-procedure.

Recovery after ESG

After the procedure, you may start clear liquids and remain on some form of a liquid diet for two weeks. Then there is progression to small semi-solid meals over the next 2 weeks and eventually to a regular healthy diet. Normally most patients go back to their everyday activities within three to five days after the ESG procedure. Patients will also continue to follow up with our multidisciplinary team of experts. One full year of follow-up visits with the physicians and with the nutritionist/dietician is included in our Reveal Weight Loss program.
